7783cd7a28 fw/qmod: enable modem RESET by default at boot
When the uC firmware starts, assert the (high-active) _RESET GPIOs,
which will cause the transistors to drive the mPCIe slot !PERST
low.

There is of course a short instance of time between the uC reset
until it executes wwan_perst_init(). To avoid this, a hardware
pull-up would have to be re-worked onto the _RESET[1..4] signals
on the PCBA

91a93bc5de firmware: add definition for main osc bypass when using external osc
Add new board.h definition BOARD_MAINOSC_BYPASS to configure the clock module to use an external oscillator rather than a crystal. The qmod board is one such board.

f721e69bc1 treewide: remove FSF address
Remove the paragraph about writing to the Free Software Foundation's
mailing address. The FSF has changed addresses in the past, and may do
so again. In 2021 this is not useful, let's rather have a bit less
boilerplate at the start of source files.

4996d7d634 octsimtest: Adjust VCC voltage thresholds (resistive VCC divider)
octsimtest has a resistive voltage divider in front of the VCC ADC
in order to also detect 5V.  We must make the thresholds board-specific
and adjust them for octsimtest.

9acff5ee5a make LED definitions board specific
the LEDs (2 of them) were connected to the same pins on all
boards, up to the octsim-tester.
to be able to have board specific LEDs the definitions have moved
from common to the each board.
at the same time I added a bit of documentation what the LEDs are
used for.

e2b0f971e5 set main clock for using UART at 921600 bps
UART baud rate is main clock (MCK) divided by CR*16.
The MCK values are chosen >= 48 MHz and <= 64 MHz to have a near
integer value CR for a baud rate of 921600 bps.
The end MCK frequency between simtrace and qmod differ slightly
but are close to 58 MHz.

c47fc5febf set local slot LED according to remote/local state.
The LED is illuminated as long as the slot is in local (physical SIM
card) mode.

3f5e3ddffc Change directory structure to align with Atmel softpack
This way we can easily check with 'diff' for differences in our code and
Atmel softpack.  Also, this layout is more suitable for building various
different firmware images (e.g. factory-test, dfu-loader, main
application) for a variety of different boards (simtrace, owhw, qmod).
